Undergraduate Notice of Proposal Form Quick Guide

The Faculty Council on Tri-Campus Policy (FCTCP) recently approved this new review stage, which must take place prior to campus academic program committee review of a 1503 undergraduate program proposal.

A unit planning to offer a new undergraduate program or make a substantive change to an existing undergraduate program must complete the Notice of Proposal review process.

Step 1: Download the Form

Download the Notice of Proposal form from the University of Washington Curriculum Office (UWCO) website.

Step 2: Provide Requested Information

Provide all requested information in the fields below. You must also include the date the form was completed.

Do not submit multiple new or program changes on one Notice of Proposal form. For example:

  • Changes to admission requirements for a BA and a BS program require two separate forms, one for the BA and one for the BS.
  • Changes to a BA program, an option, and a minor require three separate forms, one for the BA, one for the option, and one for the minor.
  • A new program and a suspension of admission to an existing program require two separate forms, one for the new program and one for the suspension of admission to an existing program.

Multiple changes to a single program only require one Notice of Proposal form. For example:

  • Changes to the admission and program requirements for a BA program.
  • Changes to the name of and requirements for a minor.

Step 3: Provide Summary and Rationale

Provide a 1-4 sentence summary and rationale for the new program or substantive program change.

Step 4: Obtain Approvals/Signatures

Obtain the approvals/signatures appropriate for your campus through its approved workflow process. Ensure each signature line includes the signer’s printed name, signature, and signature date.

Step 5: Send to the UW Curriculum Office

Send the original, signed Notice of Proposal form to the UWCO.
